With the onset of the fertile period, the uterine tissue is regularly renewed every month and prepared for pregnancy. This is called the menstrual period in women. This process is one of the most important processes of fertility. The uterus is the female genital organ where the embryo formed after fertilization attaches and grows and develops until birth. Stating that the uterine veil is the most common problem in the group of intrauterine deformities, Gynecology Obstetrics and IVF Specialist Op. Dr. Onur Meray continued as follows. The uterine septum, known among the people as 'Uterine Curtain', (intrauterine veil), is a congenital anomaly of the uterus, and is the name given to the division of the uterine cavity into two from top to bottom by a wall or curtain. He said that this excess tissue in the uterine cavity is important because it narrows the inner volume of the uterus and is among the causes of infertility.
How is it understood?
In the normal process, women may not be aware of this situation because it usually follows a symptom-free course. They are mostly informed first after applying to the obstetrician. Although the uterine septum will give symptoms, it can often appear as postmenstrual spotting or irregular menstruation. In this disease, the diagnosis is easily made with trans-vaginal ultrasound (TVS) (bottom ultrasound) when applying to the gynecologist, but to clarify the diagnosis, Hysterosalpingography (HSG) i.e. uterine film may be required. Dr. Onur Meray continued as follows; “In these procedures, the uterine septum may be a partial extension in the intrauterine area, or it may sometimes extend completely in the intrauterine area and even extend to the vagina. Therefore, besides the evaluation of ultrasonography and uterine film of the patient, vaginal examination is also important and essential. Thus, whether there is a septum, that is, the veil, including the vagina, and the cervix (cervix) can be evaluated clearly.
Are there any risks?
The presence of a septum/curtain in the uterus is not the only cause of infertility, but increases the risk of miscarriage/premature birth due to the narrowing of the intrauterine volume in the pregnancy that occurs. Previously, only late miscarriages were associated with the septum, but today it is accepted that it can also lead to early miscarriages. In addition, in the presence of perine in the uterus, there may be position anomalies of the baby in the uterus, an increase in the possibility of butt presentation, that is, the breech presentation, and therefore the possibility of cesarean delivery may increase. If there is a veil extending in the vagina, the patient may complain of painful sexual intercourse due to the narrowing of the vagina.
What is the treatment?
Treatment for uterine and vaginal septum is surgery. In the light of the evaluations, treatment in the uterine septum can be done by hysteroscopy alone, that is, by removing the septum by entering the uterus with a camera, or by entering the camera into the abdomen with laparoscopy together with the surveillance and intervention of the uterus from the outside. If it is accompanied by the vaginal septum, the chamber is cut and removed in this session. Depending on the operation, the patient can expect normal pregnancy after 1 month or 2-3 months after the surgery, or start in vitro fertilization treatment.
